- Submit the Parents’ Financial Statement (PFS) to SSS. CFS prefers you submit the PFS online. If you would like a paper version, please call our office. The due date is still January 15th. Mail your paper version to:

- Copy of final pay stub(s) for 2009, showing year-to-date earnings and pre-tax benefits.
- Copy of all 2009 W-2 earning statement(s) for working adults in the household.
- Copy of a signed, completed 2008 federal tax return, including all schedules.
- Draft copy or final copy of a signed, completed 2009 federal tax return, including all schedules. Please note: awards are contingent upon receiving the final copy of your 2009 tax return.
- Signed , but not dated, form 4506-T
- SSS Business/Farm Statement (if applicable) if you are self-employed, own a business or a farm.
- Copy of disability documentation and/or social security benefits (if applicable).

- Tuition assistance decisions are mailed to new students meeting our deadlines. Grant offers must be signed and returned to CFS by April 10, 2010.

| April 15, 2010 |
- Final, complete, signed 2009 federal tax return deadline. If you have filed for an extension, please submit IRS form 4868.
- The tuition assistance committee reserves the right to recalculate a financial aid award if information received or missing from the final 2009 tax return warrants another review.
